How to Refer

Select Level of Care
Review the comprehensive category list to identify the appropriate level of mental health support your patient requires, from outpatient therapy to intensive residential care.

Submit Referral Form
Click the 'Start Referral' button for the chosen service. You will be redirected to a secure, HIPAA-compliant external form where you can submit patient details safely.

Patient Care Coordination
Once submitted, our clinical staff and designated provider network will initiate outreach to the patient to manage the clinical intake and scheduling process.
